You can add a new field (called msgoptions) just above the Message field and use the annotate.py custom module. Operators can choose to use this, in which case the message option will be written into the Message field (together with any other message information thet they enter in the message area), when they confirm the add. If they choose not to use it, then they can write their own message into the Message area and there will be no Message option component.

You can add new message options by going to the configure link, click on the categories link and then click on the msgoptions Options (just below the Modify Category button). Now you can click on the add option button and enter the new options in the list.
